Articulation of Concern Form (for s, lc, p, m) Use this form to report complaints or concerns to the director or assistant director. Class Comprehensive Credit Request Form (for s) Use this form if you want to take a comprehensive exam in order to earn credit for a class. Not all classes apply. Early Assessment of Student Form (for lc, p, m) Use this form to describe your student's progress after the first two weeks (but no later than the third week) of the term. Enrollment Form (for s) Use this form to enroll. You must enroll before you can take a class from GROVE. This form applies to independent study classes, ASC classes, and BA and MA classes. This form needs to be filled out and submitted only once. Exam Proctor Form (for s) Use this form to record your exam proctor's information. Send this information when you submit your exam. Proposal of Class Form A (for s) Use this form to describe a class you would like to take. Potential students who simply wish to take a class on a topic of their interest and who have no designs as yet to enter a specific plan of study should use this form. You must submit this form and receive approval for it before you can submit the Proposal of Class Form B, which is the second part of the proposal. All classes are to be 8 weeks long. For this form, a learning coach, a preceptor, or a mentor will be referred to as an instructor.
